DRG-Evaluation Project ENT-Medicine, Analysis of Ear Surgery in the G-DRG-System 2005

For further differentiation of ENT-Medicine in the G-DRG-reembursement-system, the German association for ENT-Medicine (DGHNOKHC) and the ENT Medical Professional Association (Berufsverband) of German ENT-doctors, in cooperation with the DRG-Research Group of the University Hospital of Muenster, have conducted a DRG-Evaluation Project for ENT-Medicine. In this context, ear surgery was evaluated.
The aim of this evaluation was to analyse the medical and economic complexity of ear surgery and to verify its proper presentation within the G-DRG-System 2004.
Method: Retrospective analysis of the official DRG-data records (§21 KHEntgG) of 85,971 cases from 2003 from 40 ENT institutions (14 university hospitals, 16 urban hospitals and 10 other institutions). Prospective collection of data of 25,006 cases, including defined expenditure data within a period of four months in 2004. Detailed analysis of ear surgery on the basis of 1,854 prospectively collected data records with surgical procedures on the ear.
Results: Operations on the ear lead first and foremost to the non-specific DRGs D06Z and D30Z in the G-DRG-system. The analysis of expenditure on staff, length of hospital stay and material costs in the random sample presented makes it possible, on the other hand, to identify case constellations whose current presentation in the G-DRG-System is not appropriate.
By means of targeted modifications of the system of classification, the quality of the presentation of surgery on the ear in the G-DRG System could be further improved.
The recommendations for adaptation, based on the results of the evaluation project for the further development of ear surgery in the G-DRG-System, are presented.
