Objective

In contrast to the LASEK procedure the Epi-LASIK allows mechanical separation of the epithelium from the bowman layer without alcohol. In a pilot study we investigated epithelial disk ("flap") parameter intraoperatively after cut with the new Centurion SES microkeratome.

Methods

22 eyes of 11 patients, mean age 35 years were operated. Mean spherical equivalent was -3.3 dptr. (from -1.25 to -5.25 dptr.); mean astigmastism was -1.1 dptr. with a range from -0.25 to -2.5 dptr. Corneal thickness was measured directly before epithelial separation and after lifting the "flap" using the DGH 500 pachymeter (Fa. DGH Technology Exton, USA); the "flap" thickness was then calculated by substraction. The "flap" diameter and hinge width was measured with a special instrument, adapted at the curvature of the cornea.

Results

The mean "flap" thickness valued 56.7 μm, ranging from 41 to 78 μm. In all cases an epithelial flap was separated, in no apparent case bowman layer was cut. The hinge width valued from 4.5 to 9.5 mm, mean 7.2 mm. In 4 cases a free "flap" occured, which could be exactly repositioned. "Flap" diameter ranged from 7.5 to 12.0 mm, mean 10.0 mm.

Conclusions

In the 22 myopic eyes Epi-LASIK, separation of the epithelium with the Centurion SES microkeratome was performed without complications. In order to achieve less variety in "flap" diameter, an individualisation of the mircokeratome ring size seems necessary.
