Objective

Since 1980 keratoplasties (Kp) performed in Germany (G) are registered at the Department of Ophthalmology in Erlangen. The aim of this study was to find out which trephine systems (Ts) were applied in G.

Methods

For the year 2002, 122 institutions (36 of Federation of Ophthalmological Professorships (FOP), 59 of German Ophthalmological Chief Surgeons (GOCS) and 27 of Federation of Ophthalmological Clinics (FOC)) were asked for the number of penetrating (p) Kp and lamellar (lam) Kp, as well as for Ts used in donor (D) and recipient (R).

Results

We got 119 (97.5%) feedback's (FOP 36; 58 GOCS; FOC 25). In 2002, 4583 pKp and only 152 lam Kp (FOP 2644/78, GOCS 1544/38, FOC 395/36) were carried out in G. Six Ts and multiple varying Ts combinations were applied for D and R trephination. (table) GTS was favoured by GOCS (51%) and FOC (83%), followed by Barron (16%/7%) and hand-held trephine (13%/6%). The FOP institutions prefered the hand-held trephine (29%), while the proportion of the other 5 Ts ranged between 11% and 16%. In 19% of pKp different Ts were applied for D and R. This was not true for any of the FOC units.

Conclusions

Members of FOP perform the highest proportion of Kp in G (58%). Nowadays, the mostly used Ts for pKp is the GTS followed by hand-held trephine and Barron. Thus, a welcome trend towards application of the same Ts from the epithelial side in R and D is documented. Only 2 centers in G employ an excimer laser for non-mechanical trephination.
