Objective: The Total Evolutive Shoulder System (Biomet) is a new shoulder arthroplasty system in which the metaphyseal fixation is obtained with a spider-like, hydroxyapatite coated implant in cases where stemless fixation of a standard or reversed shoulder prosthesis is possible (some cases of rotator cuff tear arthritis and almost all cases of osteoarthritis of the shoulder). In cases where a stem is needed (fractures) the system provides the possibility of using a stem, combined with the ealier mentioned metaphyseal implant.

Methods: A total of 75 patients was operated using this system and after a follow up of 6 months to 3 years patients were judged on pain relief and functional gain. Also radiological evaluation was performed with emphasis on osseointegration and stability.

Results and conclusions: Good results were obtained in terms of operation technique, fixation of the prosthesis, union of the tuberosities in fracture cases and function, pain relief and satisfaction of the patient. In one case the screw connection of the prosthesis broke, one infection ocurred.
