Objective: Vaccination with dendritic cells (DC) is a promising immunotherapeutic approach for high-grade glioma. Therefore, we have set up a protocol for clinical scale vaccine generation and applied these vaccines in three glioblastoma patients.

Methods: Monocytes were enriched using the CliniMACS system. Successive generation of immature and mature DC was performed in Teflon bags under serum-free conditions, using GM-CSF and IL-4 for differentiation and GM-CSF, IL-4, IL-1β, IL-6, TNFα and PGE2 for maturation of DC. Lysates of necrotic tumor cells obtained from ALA-aided surgery served as source of tumor antigens. Vaccination was performed by intradermal injection of vaccines close to cervical and axillary lymph nodes.

Results: From leukapheresis products containing 1.3-1.5x1010 leukocytes with a CD14+ monocyte frequency of 22.1-27.7%, monocytes were enriched to 95.0-99.7% CD14+. Recovery of monocytes was 67.7-84.2%. After 6 days of culture, immature DC were loaded with tumor antigens. Selected tumor samples were of high purity (≥90% tumor) and 3.4-15.7 mg tumor proteins were processed (loading-concentration of 6.7-69.8µg/106 DC). After 9 days of culture, vaccines were harvested. 79.2-89.3% of cells displayed the mature DC CD83+ phenotype and expressed typical co-stimulatory and HLA-molecules. Recovery was 19.4-28.0%, yielding 5 vaccines of at least 1x107 cells. Vaccines were either administered directly or frozen until use. Three glioblastoma patients received a total of 12 vaccines. Vaccination was well tolerated. Immunological monitoring revealed tumor-antigen specific T-cell responses in one patient. Two patients progressed and died. One patient, having received vaccination between concomitant and intermittant parts of the Stupp scheme, is still alive and without sign of disease recurrence (day 529 post surgery).

Conclusions: Early results from vaccination of glioblastoma patients with tumor-antigen loaded DC suggest feasibility of large-scale DC generation as well as of induction of anti-tumor immune responses.
