Objective: Perineuriomas are rare benign peripheral nerve sheath tumors, which are integrated within the WHO classification system since the year 2000. They are separated into intraneural perineuriomas and soft tissue tumors. Intraneural perineuriomas were previously known as localized hypertrophic neuropathy. Because of their scarcity there are only case reports in the literature. Between 1992 and 2006 four patients suffering from intraneural perineuriomas were treated in our hospital. The patients were males and five, ten, twenty and twenty-nine years old. One of the tumors occurred in the ulnar and in the peroneal nerve and two of them in the radial nerve.

Methods: Examinations were performed clinically, electrophysiologically and by pre- and postoperative MRI. They were analysed retrospectively. Two of the four patients were previously treated with decompression and neurolysis of the nerve under the suspicion of a nerve compression syndrome. In one patient a nerve biopsy was previously performed in another hospital. Revisions were necessary following postoperative progressive neurological deterioration. Explorations of the nerves showed nerve tumors. The tumors were resected and nervegrafting was performed.

Results: The predominant symptom was a slowly progressive paralysis. Long-term follow-ups were available in three patients. No relapse could be observed. Two of the four patients showed a partial improvement of the motorial and sensorial nerve deficits in long-term follow-ups following complete tumor resection and interpositional autologous nerve grafts. In one case a postoperative neuroma was removed and a new nervegrafting was performed.

Conclusions: Intraneural perineurioma is a rare rare clinical entity. The results of this study confirm the published datas. These tumors tend to affect nerves of the upper extremities and children or young adults. Maybe there are more males than females afflicted. In cases of slowly progressive motorial neurological deficits of a peripheral nerve in young patients the differential diagnosis should include the intraneural perineuriomas. Long-term results are collected in three patients. In these cases no relapses and a partial improvement of the neurological deficits were observed.
